Transplant Guardian Angels

As a result of shadowing, the UPMC Montiefiore & Presbyterian Transplant teams realized information did not always flow easily from Care Givers to patients and families.  The Transplant Working Group developed a solution in the "Guardian Angels" program.  Made up of nursing and social work students, the "Guardian Angels" serve as more than just patient navigators, taking on the role of clinical liaisons.  They partner with Care Givers and with patients and families to always provide accurate, real-time updates and clinical information, reducing anxiety and transcending the silos that are inherent in care delivery teams, including those that commonly exist between pre-hospital, hospital, and post-hospital Care Givers.
